I notice that pearlscale is very sensitive to water cleaniness, once water dirty, the fish is exposed to bacteria easily...

This is the common problem for pearl. Usually when you get this your pearl will not live long. I suspect is internal and could be dropsy related (just that you not able to see with pearl). Fish will start to bloat and pearl drop. I don't think there is any cure but you can try antibiotic type treatment.
