A few scenerio...

1. Never change water and filter never clean
2. Change water but filter never clean
3. Change too much water but water conditioner never add enough or fast enough for the water conditioners to neutralise the harmful chemical in there

My suggestion is do a random check is filter is choked, if so change wool change water and then add more water conditioners next time. Gradually over time it should improve. A good bag of active carbon in there will help too.

Raise PH to 7.5 will also assist in healing.

Each tank is different, some accumulate more waste and therefore require more frequent cleaning. The idea is to keep the tank and filter wool as clean as possible.
I found the food and tankmates also contribute to the amount of waste appearing in the filter wool and varying the food can help to reduce bits of leftovers in the wool.

For my case, most likely is the 2 ray inside.Find them producing lot of waste, eat lots of mp then only change wool monthly result to that altho partial wc also insufficient.

Do replace the mechanical filter media i.e. the white cotton wool weekly.

Hi,

For the mechanical wool, normally I'll wash after one week, and then throw on the next weekly water change. Is this ok?
