It is probably his medication that did the big change. "Pimples" wont do much to that extent. What you can do is check your water parameters, if you do not have any test kits, just simply do a partial water change. Monitor other symptoms. Note the rate of gills movement and it will tell you exactly the health/immunity of the fish.

Yap agree that it is a hardy fish and just be very careful of what you add next time. Sad to now it RIP.

The structure of the body do not change overnight. As Kenny said, it would be muscle contractions. Well though I have not seen something that bad, there are probably a few ways around it I can think of.

It would be good that you can tell us what kind of product you used and what dosage as it will be a good reference to many who intend to use it.
