Molting difficulties, may not be a top reason, but I have experienced that, as well as attacked during/after molting.

many variables depending on whether new or old setup.
but for me, i will check these 2.
1. ammonia
2. pH

ammonia = new tank syndrome, BB die (filter issue, WC without conditioner), too high bioload, rotting plants/fish, etc

pH = crash due to lack of buffer (KH too low). pH too high due to media, tank ornaments (rock), etc.

all other variables like temp, nitrates, KH, GH etc, are secondary as these do not kill as fast as ammonia & pH problems.

Quite impossible to know why it died...unless you look under the microscope to check for diseases brought about by poor water conditions.
As such , some of the cases stated are related.
